Before you complete this form, please understand the following criteria:

• This process will take two to three weeks

• You may not be able to read the actual letter

• This form needs to be personally delivered (please do not leave on desk or in mailbox)

Also, consider the following:

• Letters are written on a first-come, first-served basis

• I will write 5 letters per month (October-February)

• After February, I will be happy to accommodate everybody else
